Do I qualify for ACA premium tax credits?

Households earning between 100% and 400% of the federal poverty level are generally eligible for premium tax credits. The American Rescue Plan expanded credits to higher income levels. What is the difference between a deductible, copay, and out-of-pocket maximum?
